天天都有排便,所以一定沒有宿便問題?錯!

有排便並不等於沒宿便,絕大多數的人都有宿便,所謂的宿便,在腸道中長期停滯淤積的陳舊大便,一般3—5日不解大便,而停留於腸管內的糞塊叫「宿便」,是體內毒素已經累積至一定程度而衍生的結果。

 

              

 

臨床上長期便秘,可使糞便內的有害物質被腸道反覆吸收通過血液循環到達人體的各個部位,會對人體造成一定的損害,如紅斑性狼瘡,痤瘡,亦導致面色晦暗無光、皮膚粗糙、毛孔擴張、腹脹腹痛、口臭、痛經、月經不調、肥胖、心情煩躁等症狀,毒素蓄積到這個地步,需要藉由「排宿便」,才能清除滯留腸道中的糞便。

 

常見的治療改善方法與優缺點:

 

一西藥軟便劑

「人生三大事就是飲食、排泄以及睡眠」,但多數人卻常因飲食習慣不良、壓力大、日常作息不正常或是天生體質的關係,無法正常排便。問題看似不大,但卻困擾著人,甚至需要長期依賴藥物來處理便秘的問題。

患者也常提及:為何已經配合多吃蔬果等高纖維食物、多喝水以及多運動等方法,卻還是無法有效解決便秘的問題呢?只好無奈地靠軟便劑來排便。

優點:用西藥軟便劑或是瀉劑來治療便秘問題或許很方便;短期服用西藥軟便劑確實可以達到98%的緩解便秘效果。

缺點:長期服用卻可能造成腸胃道功能出現鈍化或機能低下的現象,造成大腸肌肉無力,蠕動、排出廢物的力氣會隨之衰退,需要服用更高劑量的軟便劑才能緩解便秘,甚至最後縱使增加軟便劑的劑量也無法緩解便秘,腸內壞菌也會逐漸增加。

建議:許多人的便秘原因在於腸胃道功能的問題,因此治療便秘時一定要先調整腸胃道功能。

 

二酵素與益生菌

1.什麼是「益生菌」?

人體腸道中的菌叢生態同時有好菌與壞菌,而好菌就是益生菌,而年齡增長、飲食習慣改變、壓力等因素,都會影響到好菌的多寡。

不同菌株的益生菌也有著不同的效益,有的顧腸胃道,有的改善過敏或保護肝臟,因此可以針對個人不同的需求來挑選適合的菌種。

而我們常聽到的「乳酸菌」,其實就是益生菌的一種,根據乳酸菌協會的文章中指出,乳酸菌因無法在腸道中久駐,所以要「天天攝取」,以增加腸胃中的好菌數量,另外,乳酸菌也建議在「餐後攝取」,因為用餐過後,胃部的酸度較低,乳酸菌較能保持活性通過胃部,到達腸部。

 

2.什麼是「酵素」?

酵素又稱為「酶」,能夠協助將澱粉、蛋白質、脂肪這類的「大分子」分解成「小分子」,以利於人體吸收。而許多水果中便含有酵素,像是鳳梨中的「鳳梨酵素」、木瓜中的「木瓜酵素」,都可以幫助我們腸胃消化。
通常在「均衡飲食」下,身體都能自行分泌消化酵素供人體使用,但假如青菜蔬果攝取的不夠,或飲食不節制餐餐大魚大肉而引起「消化不良」,就需要額外補充酵素了!

需要特別提注意的是,益生菌與酵素在治療便秘的患者差異性很高,根據臨床上的觀察,大約只有 30%~50%的患者服用益生菌與酵素可以緩解便秘,可能與患者的體質有關。

“chronic constipation” can be categorized as a symptom of certain illnesses, or it can be an illness in and of itself. Having a daily bowel movement mean that chronic constipation isn’t a problem, right? Wrong!

    

 

 

Having a bowel movement does not necessarily mean chronic constipation will not happen. The vast majority of people experience chronic constipation, a condition during which stools sit in the intestines and do not move for 3-5 days; to a certain extent resulting in a build-up of toxins in the body.

 

Clinically, long-term constipation may cause toxic substances in the stools to be repeatedly absorbed by organs in the body, causing substantial harm, such as spiloplaxia, lupus erythematosus, acne, which leads to symptoms including a dusky or dark complexion, pachycosis, enlarged skin pores, abdominal distension and pain in the stomach, halitosis, dysmenorrheal, menoxenia, obesity, and irritable feelings. When it gets to this point, it’s time for “stubborn stool removal” to cleanse the intestinal tract.

 

The pros and cons of common remedies

I. Western medicine: stool softener

 

 “In life, three important things matter:  eating, sleeping, and having a bowel movement.” But most people cannot have regular bowel movements because of poor eating habits, stress,  irregular sleep, or innate physical issues.  The problems don’t seem to loom large, but they are nevertheless annoying,  even leading to dependence on medications for treatment. Patients often  ask: Why is it that I eat a variety of fruits and vegetables high in fiber and drink plenty of water and do exercise, but still cannot effectively deal with constipation? I just have to resign myself to taking stool softeners.

Pros: Using Western stool softeners or laxatives may be convenient; short-term use of these stool softeners does indeed attain 98% efficiency.

 

Cons: Long-term use conversely may create passivation or deterioration of gastrointestinal functions,  and cause weakened intestinal muscles, gastric paristalsis,  which in turn leads to a decline in strength to excrete waste matter from the body, and the need to take higher doses. Eventually, even high dosages won’t ameliorate the situation, and bad bacteria in the intestines will gradually spread.

 

Suggestions: For many constipated people, the problem lies in the gastrointestinal tract. The first thing to do then, is to regulate gastrointestinal functions.

 

II. Enzymes and probiotics

 

1.What are “probiotics”?

 

The ecology of the bacterial flora in the intestines includes both good and bad bacteria. The good bacteria are called probiotics; factors such as aging, changing of diets, and stress all negatively impact the number of good bacteria. Different strains of good bacteria  are beneficial in different ways. Some mind the gastrointestinal tract, others mitigate allergies or protect the liver.  For these reasons, select different strains of bacteria to suitably match individual needs. One of the probiotics we commonly hear about is lactobacillus  (lactic acid).  As indicated in a paper from the Taiwan Association  for  Lactic Acid Bacteria,  lactobacillus must be “consumed daily” to increase their numbers, because these good bacteria do not remain long in the gastrointestinal tract.  In addition, it is suggested that lactobacillus be consumed after meals, because these bacteria remain more or less active as they pass through the stomach into the intestines.

 

2.  What  are “enzymes”?

 

Enzymes have the ability to accelerate the decomposition of protein, starch or fat, to facilitate absorption by the human body. Many fruits have enzymes, such as bromelain in pineapples, papain in papaya, that can aid gastrointestinal digestion. Ordinarily, with “balanced meals,” the body can secrete digestive enzymes by itself for the body to use, but supplementary enzymes are called for when consumption of fruits and vegetables is insufficient or when indigestion occurs because of overeating and feasting.


It is very import to note that there are major differences in patients treated for constipation with probiotics and enzymes.  Based on clinical observations,  only 30%-50% of patients taking probiotics or enzymes find relief. This may have something to do with differences in physique.

 

A suggestion worth trying: a mix of structured c-peptides extracted from natural herbal plants     (containing amino acids and protein) can promote peristaltic waves in the large and small intestines, increase secretions in the digestive tract, strengthen the function of digestion, eliminate pneumatosis in the digestive tract, and aid in the excretion of waste matter from the body.
